Dear Parent/Guardian,
Nebraska state law requires that students have a physical examination conducted by a physician, physician assistant, or advanced practice registered nurse-nurse practitioner within the six months prior to entrance into the 7th grade. If your child received the school sports physical on May 13th, the physical requirement has been met. In addition, all 7th grade students must provide the school with evidence that they have had a booster dose of a tetanus, diphtheria, and pertussis (Tdap) vaccine prior to the beginning of school.
Failure to have these requirements completed will result in your student being excluded from school until the requirements are met. Exclusion from attendance for non-compliance will begin on September 7th, 2021.
You can make an appointment for a physical examination by calling the Chadron Medical Clinic at 432-4441. Immunization requirements can be completed at the Dawes County Immunization Clinic (DCIC). You can call 432-8979 to make an appointment. The DCIC is open on the first and fourth Tuesday of each month.
